Job Summary

The Case Manager will provide case management and chart management support to clients within the Goldman Clinic Methadone Maintenance Program . This role requires interaction with staff and clients to ensure the highest quality of service and compliance with licensure and accreditation requirements .

Responsibilities
  • Assess, develop, implement, and monitor comprehensive service plans for each client.
  • Facilitate documentation requests for Social Security and Department of Welfare applications.
  • Assist with client transfers in and out of the Methadone Maintenance Program.
  • Maintain an up-to-date community resource and referral database.
  • Monitor treatment progress and coordinate access to necessary support services, including housing, job training, and healthcare.
  • Ensure proper insurance coverage and funding authorizations for services.
  • Identify clients at high risk of leaving treatment and implement retention strategies.
  • Lead one orientation group per week to help new clients understand program policies.
  • Work closely with counselors, therapists, and other service providers to ensure clients receive appropriate care.
  • Provide crisis intervention and referrals as needed.
  • Maintain compliance with confidentiality and regulatory standards.
Qualifications
  • Education: Associate degree or higher in a related field from an accredited institution.
  • Experience:
    • Experience in substance abuse or mental health case management preferred.
    • Knowledge of community resources and public insurance systems.
  • Certifications:
    • Current First Aid & CPR certification preferred.
    • Bilingual (English/Spanish) skills are a plus.
Skills & Abilities
  • Strong knowledge of psychiatric disabilities, substance abuse issues, and treatment approaches.
  • Excellent documentation, writing, and organizational skills.
  • Ability to multi-task and prioritize responsibilities effectively.
  • Strong interpersonal and crisis intervention skills.
  • Proficiency in Electronic Health Records (EHR) and Microsoft Office Suite.
  • Ability to work collaboratively in a fast-paced, multidisciplinary team environment.
Work Environment & Physical Demands
  • Requires standing, walking, and interacting with clients for extended periods.
  • May involve lifting, maneuvering, and transporting materials.
  • Must be comfortable working with individuals in potentially high-stress or crisis situations.
  • Potential exposure to environmental hazards and infectious diseases.
  • Flexible adaptability to program needs and scheduling.
