Job Overview:
We are seeking a dedicated and detail-oriented Clinical Pharmacist to join our team, specializing in home infusion therapy. This role is responsible for assessing patients' suitability for home care, developing individualized pharmacy care plans, and ensuring compliance with state, federal, and USP regulations. The ideal candidate will have experience in sterile IV admixture procedures, medication therapy management (MTM), and patient education.
Key Responsibilities:
Patient Assessment & Care Planning
* Evaluate patients' medical conditions and prescribed medication therapy to determine suitability for home infusion services.
* Develop and recommend individualized pharmacy care plans, ensuring appropriate medication selection, monitoring parameters, and treatment goals.
* Perform nutritional assessments and make appropriate recommendations for patient care.
Medication Management & Dispensing
* Prepare and dispense sterile and non-sterile compounded medications following Option Care policies and USP standards.
* Ensure medications are accurately labeled, properly stored, and include all necessary ancillary information.
* Maintain compliance with Drug Enforcement Administration (DEA) and controlled substance regulations, including tracking perpetual inventory systems.
* Monitor medication administration equipment, troubleshoot issues, and educate patients and caregivers on proper use.
Clinical Oversight & Compliance
* Conduct quality assurance reviews to ensure safe medication practices and compliance with accreditation and regulatory requirements.
* Participate in incident reporting and medication error management to continuously improve patient safety.
* Collaborate with physicians, nurses, and other healthcare providers to ensure continuity of pharmaceutical care.
Education & Professional Development
* Provide medication-use education to home care patients, caregivers, and Option Care staff.
* Stay current with continuing education requirements and attend relevant seminars to maintain professional competency.
* Respond to drug information requests from healthcare providers and patients, utilizing evidence-based resources.
Qualifications & Requirements:
Basic Requirements:
* Pharmacy degree (PharmD or BSPharm) from an accredited school of pharmacy.
* Active and unrestricted Pharmacy license from the State Board of Pharmacy.
* One of the following qualifications:
* At least 1 year of experience in sterile IV admixture procedures.
* Knowledge of non-sterile compounding, immunizations, and medication therapy management (MTM).
* Completion of an ASHP-accredited Pharmacy Residency Program.
* Completion of an Advanced Pharmacy Practice Experience through an accredited school of pharmacy.
* Equivalent experience approved by the Vice President of Pharmacy or Senior Vice President of Clinical Services.
* Must obtain Board Certified Oncology Pharmacist (BCOP) certification within 6 months of hire if not already certified.
Technical Skills:
* Proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
* Experience using electronic health records (EHR) and pharmacy management systems.
Preferred Qualifications:
* Completion of an ASHP-accredited Pharmacy Residency Program or Advanced Pharmacy Practice Experience.
* Experience in home infusion, hospital pharmacy, or a clean room setting.
* At least 1 year of indirect team management experience, including training and overseeing work assignments.
