We're looking for dedicated Advanced Practitioners to join our integrated care team, which includes a Registered Nurse and a Social Worker. You'll play a vital role in providing personalized, compassionate medical care to individuals with chronic kidney disease (CKD) and/or end-stage renal disease (ESRD/ESKD). This position offers a flexible schedule with a hybrid and in-home care model, allowing you to connect with patients in a meaningful way.

What You'll Do:
Conduct comprehensive assessments, including annual wellness exams, both in patients' homes and virtually.
Counsel and educate patients and families about available benefits and programs to support their health.
Maintain detailed and accurate patient records, including chief complaint, diagnoses, history, reviews of systems, examinations, medications, allergies, assessments, and plans.
Coordinate care with primary care providers, specialists, and ancillary services.
Complete all documentation and paperwork promptly.
Maintain high standards of care.
Attend periodic training and meetings (may require out-of-state travel).
Deliver evidence-based, timely care that minimizes hospitalizations, maximizes quality of life, and prioritizes patient health and satisfaction.
Prescribe medications, order tests, and collaborate with patients' physicians.
Contribute to improved patient outcomes through quality measures and reporting.
Facilitate closing gaps in care by educating patients about preventive monitoring and scheduling diagnostic testing.
Assist patients with accessing educational resources, such as videos.
Participate in integrated care team meetings.
Make assessments of patients' health status.
Develop and implement treatment plans.
Follow up and evaluate patients' status.
Perform other duties as assigned.
What You'll Need:
Bilingual (English/Spanish) skills are highly preferred.
Active and unrestricted Registered Nurse and Nurse Practitioner or Physician Assistant license.
Board certification (NP: ANCC/AANP; PA: NCCPA).
Current and unrestricted DEA certificate.
Ability to work independently and autonomously.
Access to transportation, a valid driver's license, and car insurance.
Proficiency with medical instruments and equipment.
Knowledge of computer-based data management programs, information systems, medical records, and point-of-interview technology.
Excellent communication skills (verbal and written).
Understanding of federal and state regulations, including HIPAA and OSHA.
Two or more years of direct patient care experience.
Experience with managed care/IPA/health plans.
Experience conducting annual wellness visits or similar comprehensive visits (virtual or in-home).
